This Webinar will examine the opportunities for collaboration with health plans to provide palliative care services for their seriously ill beneficiary population. Key concepts include identifying high risk individuals, core team members and interventions, measurement and metrics to demonstrate value and reimbursement models. Considerations include how data is shared, frequency of communication, program cost, methods of program evaluation and contract negotiation.
