Description

Beyond a disease approach, the “Experience Approach” to care is grounded in an interdisciplinary model driven by patient/family values, goals and wishes for how they want to live out their lives. Whether the patient has nine hours, days, or months to live – an interdisciplinary team (IDT) that continually focuses on what is most important to the patient and family will help to create transformative experiences. This Webinar will discuss components of creating and maintaining a culture where hospice interdisciplinary practice thrives, supports patient and family values, goals, and wishes, and enriches the lives of staff and volunteers.
 
Objectives
At the completion of this program, participants will be able to:
•Describe the specialty of hospice interdisciplinary care that focuses on transforming patient/family end-of-life experiences
•Identify components that create and hardwire an effective interdisciplinary team culture, including IDT meetings
•Demonstrate the value of interdisciplinary practice for patients/families, staff/volunteers, and the hospice organization
