Comprehensive assessments and the plan of care are more than regulatory requirements, they are the foundational components of hospice care. Done well, they provide a roadmap of care provision and the ability to ensure the care is making a difference. The OIG gave us a wakeup call this past July by highlighting the ongoing survey deficiencies related to these areas and how they effected the patient’s care. This Webinar will address the regulatory requirements and the importance of understanding the needs of patients and families in assessing and developing an ongoing plan of care that makes a difference for patients and families at end of life. One hospice’s experience in modifying their EMR’s provided care plans to provide individualized disease focused care plans will be shared along with other resources.
