Having a steady stream of an adequate number of referrals is a prerequisite to success (and survival) for any palliative care program. In most cases, success in this area depends on developing relationships with a range of
individuals and organizations that provide or pay for clinical and social supports and services to seriously ill individuals who might benefit from palliative care. In this Webinar we will review strategies, promising practices
and lessons learned for promoting appropriate referrals, information that could be incorporated into your referral promotion strategy.
Learning OutcomesAt the completion of this program, participants will be able to:
•Appreciate variables that promote or impede referrals to palliative care
•Identify examples of organizations that might be included in a patient identification / referral network
•Describe a process for incorporating palliative care screening and referrals into the workflow for a potential referral partner
